After 15 mins of my game one of my players was sent off. I went into the tactics screen to change my tactics, after that I clicked OK and there was no running commentary. I play the game on commentary only. The commentary bar was black and kept flashing for the duration of the game whilst the scoring was ever changing.
This is the second time this has happened in my save, anytime one of my players gets sent off this happens.

More example of ghost goals.
The game against PSG, I got a player sent off after 20 mins and the commentary bar disappeared. So couldn't read anything and the game continued. I couldn't even make a change.

Yeah I am old champ manager player too. Yeah you're right as the game is practically ending a goal gets added to opposing team majority of the time.
Hopefully when the new update is released then they can sort it because it urgently needs sorting. I've lost count the amount games that I think I've won by the odd goal then all of a sudden the other team gets a goal without any play.

I started a topic in this section called goals scored in commentary only, randomly appearing.
If you have a look you'll see I've submitted tons of saved games, so yeah it's definitely a major bug in the game. The SI team are aware of it, so hopefully in the next update it can be fixed.

This is a massive problem, been happening in my games since the game was released. You need to start saving games as soon as they're finished and submit the PKM.

Here are another 2 examples. If I can remember correctly in both games goals were scored just after half time in the 46th min. Both times the goals were scored for my team.
It's infuriating when the opposing team benefits from this and it doesn't feel much better when my team gets the goal because it's not based on any tactical play or brilliance by one of my players or a mistake from the opposing team.
I'm at the stage now that in the second half I barley read the commentary as I am focused on the scoreboard because I know there's a very good chance that a ghost goal will appear.
Lost count the amount of games that I think I've won by the odd goal only to find out the game ended all square because a ghost goal appeared. Again, there's other times that I've benefited from this as I've been losing by the odd goal then it turns out that I've scored and the match has finished in a draw. Either way it's not right.

I know there was an update on the 8th Jan or thereabouts. So I haven't come across this issue as much but occasionally it does happen and again it's always in the second half of matches.
The Spartak Moscow v Krasnodar I played on the 9th Jan. This match was crazy the score ended 4-4 but I think about 4 or 5 of the goals just appeared randomly. This was the worst match I've seen for ghost goals. The commentary bar went missing too so I just had to keep checking the scoreboard and it was forever changing.

There's definitely a bug whilst viewing a match in commentary only. I have numerous examples where goals randomly appear, also there are times when the commentary bar is black for the full duration of the second half and goals appear. So you're unable to read anything.
I have attached a lot of saved matches, so you'll be able to view those matches and see for yourself. It is always in the second half when these random goals appear.

My team were involved in a penalty shoot out which I won but never saw any running commentary of the penalties, it went straight to the other teams 5th taker and he missed then mines scored and I won. It was all very rushed and blink of an eye moment.
The game is attached. The match finished 2-2 but my second goal just randomly appeared there was no running commentary. There seems to be issue on commentary only with goals randomly appearing, I've posted numerous examples with saved matches. It always seems to happen in the 2nd half of matches.
The issue with random goals appearing, is this a bug within the game or an issue that you're trying to fix?

Onni Valakari is from Finland and is a Finnish international, he made his debut in November against France and scored. He's never represented Scotland at national level. In my save he is a Scottish under 21 players. He currently plays in Cyprus for Pafos.

Noticed on my save that when the Scotland squad is announced it's saying that the focus of the squad is foreign based. All the players play in the UK, Scottish players aren't classed as foreigners in England.

The opposing team scored after half time but there was no commentary of their goal. It just randomly appeared that they had 1 goal.
I have saved the match and attached the file below, would you be able to look into this?
St Mirren v Celtic - the exact same thing happened. I have added in more games where goals have randomly appeared without any commentary. The trend seems to be that these goals happen in the second half, mainly just after half time. I have noticed the odd few happening later on in the second half too.

Game play has became really slow/sluggish since the latest update. I do use a 3rd party skin so I ended up going back to the original skin but was still noticing the same issues .
When I click in the search bar and type in any team or player it can take about 10 seconds to find them, also when I am in my squad page and I click on any player in my team again it can take about 10 seconds just to view their profile,
Before the update the game was running like clockwork, I have a high performing PC so I don't think there's any issue from that end.

Yeah that makes sense, just because it came up flashing after a few pre season I was wondering why it was happening. I haven't started any competitive matches yet so I'll hopefully the media reaction will appear when I do so.

I have started a new save after the update but I notice that after the games in pre season there's no media reaction after the games. It comes up on the screen for a few seconds then disappears. Has this been fixed?

After I play a match the media reaction appears on the screen for 2 seconds then disappears without even clicking anything, so you don't get to see what they've said.

The semi final draw for the Euro's is incorrect. Obviously due to COVID the games were played earlier this month but the draw for the games was made last year. I am pretty sure the fixtures should have taken place in March this year. In real life the draw was Scotland v Israel, Norway v Serbia, Iceland v Romania and Bulgaria v Hungary, Bosnia V Northern Ireland, Slovakia V Rep Ireland, Georgia V Belarus, North Macedonia V Kosovo

IK Sirius play at a new stadium however in the game it's still showing the details of their old stadium.

I think there's any issue with player instructions that they know about. I am having similar issues by telling my striker to shoot more, then when I go back into edit more instructions for my striker it appears that I never told him to shoot more as it's not been selected.
You should provide screenshots to the SI guys, will provide them will evidence of what your issue is.
