This happen quite often, both on mobile or Xbox console version.
During certain matches when my players got tired, my assistant manager would recommend to substitute. Sometimes, even after I have 3 subs already, the assistant screen pop up and if I accept, I get 4 subs. In this screenshot, I got to 5 subs.
This happens when I make a sub and assistant recommends it right after.
