    The game allows a sixth substitution during extra time. In real life we only get an extra open-play substitution slot (plus the breaks between regular and extra time/between extra halves). There still is the limit of maximal five substitutions.

    Source: Last year the VfL Wolfsburg was disqualified for using an illegal sixth substitution. There was no relevant change in rules since then. The rulebook (p. 19) says the sixth substitution during extra time is an optional rule that needs to be stated in the relevant competition's guidelines. There is no such entry in the DFB-Spielordnung.
