Injured player not being subbed off after I made the change

Hi,

I've had an issue where an injured player wasn't subbed off even though I made the change. Alex Telles got injured in the 70-something minute, so I substituted him for Brandon WIlliams. This change never happened though, the player sub information just kept increasing with the time of the match, so that by the end it said Williams was subbed on in the 90+6th minute, but never actually made it onto the pitch. I've attached the pkm of the match if you can use it! Am on 21.1.1.
