It seems that 2nd teams aren't being promoted. Or at least that it happens much less often than in real life. After playing some seasons in Norway I started a new save and went on holiday for 10 years. In those ten years not a single 2nd team was promoted from 5th to 4th level. (Unless I missed someone when going through the leagues). Another thing is that the 2nd teams that are on 4th level at game start seems to have overperformed and almost all of those teams have been promoted to 3rd tier.
Compared to real life where a couple of 2nd teams are promoted to 4th level each season, and there usually are only two or three 2nd teams on 3rd level
