Other team allowed more substitutes (3-5 sub rule bug?)

Just to confirm, the rule is that you can make 5 subs but only during 3 intervals. 

For example, 1 sub at 50, 2 at 65, 2 at 80. 

1 sub on 4/5 different occasions would not be allowed.

Does that sound like what the issue could be, or is it happening even with only 3 stoppages of play?

I had no idea, this is probably what happened as the AI is making all their substitutes in batches. Thank you for the clarification
