While running some tests on an extended Polish database, I noticed that some teams (mostly in the lower leagues) were being assigned to the wrong regional divisions - in the III Liga at senior level, and the Liga U-19 at youth level.
On further investigation, I found that at least 160 of the 293 Polish teams in the database don't have their Regional Divisions settings set up properly. I've listed these clubs below, and the Regional Divisions that should be assigned to them. Apologies in advance for any misspellings.

I've based these on the local regions that the clubs are based in. Some clubs that are close to a border with another local region may not be entirely accurate, but most of them should be correct.
P.S. There is one Polish club in the database that does not currently have a city set. Zenit Miedzyborz (2000274499) should have its city set to Miedzyborz (722114).
