Steps to reproduce: (not certain this works, but seems to be the situation in which I've seen it)
If a player gets hurt for Team A and Team A kicks the ball out of bounds in their own half. When Team B throws the ball in to give it back they seem to aim at throwing it out for a goal kick. However a player from Team A tries to save the ball at the end line from going out, resulting in a corner kick when they touch just as it is crossing the end line.
I've seen this multiple times (3-5). It feels like it happens every time in these conditions. However, its completely possible I am only remembering the times I've seen it go wrong.
Somewhat related, I've seen this similar behaviour of saving the ball on the end line in other conditions on occasions but I always viewed it as players making a mental error playing a ball they shouldn't have. Not sure if playing with high mentally skilled players would reduce this from happening.
