I started a new save and it the same thing happened again, so it seems to be fairly easy to reproduce.

I had player X as a Captain and player Y as Vice-Captain. I stripped X from captaincy and promoted Y from Vice-Captain to Captain. This made player Y mad, because I stripped him from his Vice-Captaincy, even though he was made a Captain.
