I have the same issue in my save. I won two consecutive Ekstraklasa titles with LKS Lodz and I got the same Steam achievement as @PhilH82 did. Also this achievement appeared in my career milestones.
Moreover, this is not the only such case in my save. For example, Jurgen Klopp won English Premier League twice in a row, but he also has this achievement in his career milestones.
I uploaded my save to the Cloud. The save name is "Lodz".
