You need to load all players from these countries.

Some countries still won't have enough players, in which case you need to load any clubs that generate players from that country, and sim a few years.

In your case if you looking for San Marino you will need to load the San Marino update and make sure the option to load players for the teams is activated in order to have enough players.

If you run the Italian league and load all players from San Marino, you should be able to manage them.
