Looking at that, it seems a minor bug brought on by the injury at the same time as the actual subs. It's a one in a billion scenario, but congrats on getting it!

Isn't really a big issue - maybe some daft English referee decided to do an extension of his "three yellows before a red" routine ^^

Im going to hazard a guess that you made a substitution where the player hadn't been brought on yet, and you ended up getting an injury, so the game thought you still one substitution left, when in fact you'd already used it.

there are only 11 on the field for you though u didnt get a 12th man just 4 subs

Three players were subbed off the field while 4 players come on, therefore, there are 12 players on the pitch. You can even count the active, bold players (keeper counts) and you will not that there are 12 there.
