Creating a cup competition for a local region

Make a new inactive competition(the details like name etc. really don't matter, it's just a dummy for holding teams for a cup, you'll never see it mentioned in game), add all teams in that region to this comp as secondary division. Then when creating the cup, simply get the 48 best teams from that comp. -

Varying the number of teams in a qualification round

If you make a 64 team tournament as one stage, then untick "number of teams" in that stage, and set the minimum and maximum teams, it will automatically adjust round sizes to fit the number of teams, as long as you make enough rounds to cover the maximum tournament size. In the case there is less teams than the maximum teams allowed, it will give a bye to the team at the end(NOT the start, remember to enter teams in reverse order of seeding) of the team list until it has a valid set of rounds. -
League Structure with Multiple Stages

Multiple stage leagues work fine. If no one is relegated, it would be no different than making a cup except for the fact the champion must be marked(not decided, just marked, you can use fate actions to mark someone as champ in another stage) in stage 0. Just use various qualifying fates etc. to drag teams between stages and make sure to properly set up ranking levels. Could be a bit of a nightmare if you're not familiar with the advanced editor is the warning I'll give you. Maybe make a more simple 3 stage competition(for instance, Scottish Premiership) from scratch first to learn how it works. -
But according to what you wrote, there are 24 winners in the previous round. If you don't add anything to this, then you will be short teams.

You need to qualify the teams to the final using qualification rules in the group stage league settings. -
Split season division with regional sub-groups

You need to make sure to set fate actions for playoffs. Any team who is promoted or relegated must do so in stage 0 of the competition they belong to. For instance, any team relegated from Liga 1 would need a fate action to set they are relegated in stage 0 of Liga 1, and any Liga 2 team promoted will need to have a fate action to say they are promoted in stage 0 of Liga 2. In this instance, any team set to relegated in this stage will also be set to relegated in stage 0. Leaving the competition blank like I have here will make it for the competition the stage is in, but if they are from another division, you will need to set the division they belong to in the competition. -

The round set, I assume, is wrong here. The "round name" must match the name of the round the teams must win in to qualify. Then you need to set "main stage name" to the round or stage they have qualified for. For rankings, you would likely be best looking at a real tournament. A good example may be the Scottish League Cup, called Betfred Cup in my game, but may be different in yours as it uses a sponsored name. Simply load Scotland by converting the real rules then check the ranking levels for each cup round and the group stage. -

You haven't sorted your teams and you have not given them incremental seeding, therefore everyone is getting seeding 0. -
If you do not sort or reverse the teams, then the game will get them in the order of final position. Simply make sure that teams are seeded incrementally then make the draw seeded. It should not need any more configuration. If it still doesn't work, using a fixed draw flag and setting up a fixed draw of team 0 vs team 3 and team 1 vs. team 2 should work fine. This fixed draw, if you haven't set it to be two legged or neutral venue anyways, will also ensure the higher placed team will play at home.

Continental cups-Lower rated teams to play at home

Make sure it isn't a seeded draw, but make sure lower seed plays at home. Lower division will not work because all teams will be the same division level. -
Pretty sure team 0 is the first team in the list like always. I've never used this particular function, but 0 is always the first team, and not "no team".
