Has anyone ever?

yes gone through loads off matches without injuries.. but when i do get injured players it mostly 2 at a time which can be annoying... a lot off my youth players get injured in the gym but thats only the training is heavy loaded but works wonders with development...

Injuries have not been a problem for me.

If you already do this, then it is of no help, but I will post it anyway...

Keep an eye on the commentary, or screen where you can see the % of fitness, if you see a "PlayerX has picked up a knock" or something like that, check their fitness %, and if they have a faded injury sign next to them. If they do, or you see their fitness is anything lower than say 65%, I would always look to take them off ASAP! This of course depends on the score of your match, and if taking them off will damage your chances of winning an important game etc.
