Help - Xerez - I keep conceding goals from counter attacks and set pieces

How do I make the necessary adjustments?

I have check how I have conceded goals and the opposition usually scores from capitalizing from me loosing possession in midfield. Obviously defending deep would be a solution but that can't always be done and neither does it always work. I usually use a 4-5-1 deep (2 DM) or the classic legendary 4-1-1-3-1 formation (high line, play offside, compress space.)

No idea how you're 7th with Xerez as it means you're doing it correctly! How do you concede the goals? Does the opposition strip you for pace? Do the wingers rip you apart? The only thing I suggest is stopping possession from being lost which tinkers with mentality and passing style to make sure your midfield has enough time to make a pass.
