Players from top clubs, for instance, will retain players at the top clubs in whatever country/region/continent you select. So, for example, if you select to retain players at the top clubs in South America, your save will contain players for all of the top clubs in that particular continent. It's just a good way of adding extra players into your save and adding some extra depth and playability. One thing I am unsure of is whether retaining current international players in, for instance, Europe, means that all international players from European international teams will be retained, or whether it means that all international players from European clubs will be retained regardless of their nationality.
